- $25,000 for bodily injury for one person per accident
- $50,000 for bodily injury for all persons in one accident
- $25,000 for property damage per accident

This form must contain the name of the insured person, name, address and five-digit NAIC company code of the carrier, as well as the name, address and phone number for security verification.
