- It is possible to have a different address on your driver s license and your car registration. Once you submit a change of address form with the Oregon DMV, your entire record will be updated with your new address. This means that your driver information and any car registered in your name will contain the updated address with the DMV. However, you do not need to replace the physical driver s license or car registration card. At your request, the DMV issues an address sticker for you to place onto your current driver s license to update the address. There is no charge for this sticker. For your car registration card, you must write in your new address onto your current card.

- If you would like to request a name change on your Oregon driver s license, you must apply in person at the DMV and:
- Fill out a Driver License Application or the Commercial Driver License Application form, whichever is applicable.
- Provide proof of full legal name.
- Provide proof of legal presence in the United States, date of birth, and identity.
- Provide your Social Security Number.
- Provide proof of residential address, if required.

- The requirements to apply for a name change on your Oregon ID card are as follows:

- Provide proof of legal presence in the United States, date of birth, and identity

You must apply for a name change in person at the DMV. Changing your name on your Oregon ID card does not automatically change your name on your Oregon vehicle record, your title or registration. On the form to update your address, there is also an option to apply your new address to your voter registration information.
