The state of Massachusetts does not require you to present any documentation when changing your name; however, you will have to report your change of name at a RMV branch.

How do I change my name on my ID card?

To change the name on your ID card, you will need to complete the same MA name change request process as you would for a driver s license:

Visit your local RMV branch
Provide your new Social Security Card
Fill out an Class D, M, or D/M License and ID Card Application
Make a $25 payment (in cash, check or money order) for a duplicate ID card
